I know this blog post is old, however Microsoft TechNet has an article that is fairly thorough on how to do this. I recently had to redo this from scratch (on the ISA 2006 side, anyway) and by following this almost to the letter including Appendix D, I was able to accomplish the goal.

Thanks for the update! We are using TMG 2010 now, but for the most part it identical to ISA 2006 from an end-user perspective so this likely still applies.
